Our office is now Paperless.  All New and Current Patients are asked to come to our website and complete all the Patient Questionnaires.  To do this all you need is your name and appointment date.   The Questionnaires will become part of your permanent file.  We will have you sign the documents once you come in for the appointment.  All Active patients current information is being scanned into the computer where the Doctor will have access to all of your past information.  We appreciate your assistance in helping us reach this goal.
